Vista Healthcare offers professional healthcare services, including Physiotherapy and Occupational Therapy, to homecare package clients in the Gold Coast, Brisbane, and greater Sydney regions. In addition, we collaborate with approved homecare providers to provide brokered services.
The Homecare program is a government-funded initiative that allows individuals to receive assistance in their own home. The Homecare packages are classified from level one to level four, and they offer varying levels of funding based on the level of care required.
At Vista Healthcare, we are dedicated to assisting all homecare physiotherapy clients in achieving their goal of living safely, independently, and happily in their homes for as long as possible. Our physiotherapy services are specifically designed to support people and their caregivers in optimising their quality of life in their home environment.

Our residential aged care services are available to all homecare package holders. Our clinicians are mobile across the Gold Coast, Brisbane, and greater Sydney regions.
Home Aged Care Services
Vista Healthcare is a holistic provider of Physiotherapy & Occupational Therapy services. We are a mobile team of clinicians and visit people in their homes. We help with:
- Rehabilitation at home
- Assessments
- Equipment prescription
- Community access (i.e. wheelchairs, walkers, gait pattern)
- Lowering and managing pain
- Falls prevention
- Optimizing daily living
- Post-surgery rehabilitation

We are also experienced with a range of physical and neurological conditions, as Vista Healthcare’s origins began in aged care.
What is included in homecare therapy?
When we meet a person for the first time or during ongoing therapy sessions, our Physiotherapists and Occupational Therapists will visit them at home for around 1-2 hours.
They will conduct subjective and objective assessments to determine the person’s current ability, and then develop a suitable rehabilitation plan, including suggestions for assistive equipment, and track progress towards their goals.
We take a holistic approach, so we may also address other areas of life, such as managing continence and vision.
In many cases, we are engaged to help people live more safely, and to that end, we may conduct falls risk assessments and environmental reviews.
For example, to reduce the risk of falls, we may focus on
- building lower limb strength
- prescribe bathroom rails
- prescribing a shower chair
- addressing fatigue components
Our caring clinicians will work with the person in a holistic manner, from building strength to prescribing home exercise programs, trying out equipment, teaching safe strategies, and just enjoying the benefits of exercise together.
We record all activities and progress during the sessions to ensure accurate tracking.

Vista Healthcare was fortunate to help a lovely client that lived on a very big property.
The husband has advanced Parkinson’s disease and the wife had moderate to advanced dementia. Upon meeting these people, the wife was chair bound and struggled to stand without the assistance of a lifter or carer. Because of this, she spent a lot of time sitting in front of the TV.
The lady loved cooking and standing at her sink which overlooked the mountains and her lovely garden. It was her meaningful goal to develop the capacity to walk herself to the kitchen and safely cook dinner, look at the beautiful view and enjoy herself at home.
We worked together to safely practice walking. At first, it was just a few steps at a time, then a few more. Having a neurological condition, it is important to lower the risk of falling. Falling mostly occurs because someone cannot react fast enough to move their feet. So, we practice a lot of balance and sit-to-stand exercise together.
Vista Healthcare worked with the lady for several months, and in the end, the lady was able to walk to the kitchen and enjoy her beautiful home and independence. For the times where she was tired, she had access to a wheelchair to help her.

Home Care Package Client FAQ
- What does Vista Healthcare do in terms of scheduling, documentation and compliance?
In most cases, we will receive a client from a homecare provider, it is also possible for homecare clients to self-refer.
We will then perform an intake of the referral made, which is us typically phoning the referee and asking more details about the Physiotherapy/OT requirements and synergizing with the case coordinator.
We will organize our initial assessment at the persons residence to conduct an initial assessment. Vista Healthcare takes care of all scheduling. So, we will visit the client and communicate with them in the way they would prefer.
We store all clinical documentation using our practice management systems which can be provided to the homecare coordinator for their compliance, providing we are consented to do so. We value privacy and confidentiality.
In the current environment, it is important to know that we take the risks associated with COVID-19 seriously and all our staff have untaken the Department of Health COVID- 19 infection control online training for health workers in a community setting. This training covers the fundamentals of infection prevention and control for COVID-19.
- Is Vista Healthcare able to assist with all homecare package levels from one to four?
Yes.
We are experienced supporting people under all these types of circumstances. The most important thing is to ensure that we are an appropriate provider and can help people achieve realistic goals.
On the initial phone call, we will ask questions to ensure we are a suitable provider for that person.
- Can Vista Healthcare help short-term restorative care (STRC)?
Yes, we definitely can.
Short-term restorative care is a support program to help people being discharged from the hospital to their home or a respite facility. These programs typically span eight weeks in duration, and we will visit them 1-2x per week.
The common cases for STRC are people who are recovering from hip and knee replacements, which are the two most common surgeries. However, we also handle more complex cases.
Vista Healthcare is engaged to provide Physiotherapy and/or Occupational Therapy interventions to help people live as independently as possible and achieve their wonderful goals.
- Is Vista Healthcare able to perform cognitive assessments and neurological rehabilitation?
Yes.
Cognitive assessments are primarily performed by our Occupational Therapists.
There are a number of different assessment tools that allow us to measure mental capacity, function, short/long term memory, multitasking and so forth. Many of these skills translate into daily life, such as the ability to organize a schedule.
